HealthSpan Complete Care

HealthSpan’s Care Management team offers a fully-integrated approach to health and wellness management. Utilizing all opportunities for early identification and intervention, we are able to impact health outcomes and potentially enhance health care dollar savings for members. Care Management services include pre-certification, utilization review, case management, disease self-management program and lifestyle management services which include the HealthSpan E-Coach program and personalized follow-up on comprehensive health screenings.

Employee Assistance Program

Life Management Systems, a service of HealthSpan, Inc. offers a comprehensive Employee Assistance Program (EAP) to employers. The goal of the EAP is to provide a service to employees and their household members to address their emotional concerns, confidentially, with a professional counselor. The core of the program is face-to-face contact with a counselor for problem assessment, brief counseling and referral coordination. Many clients choose face-to-face counseling when struggling with emotional and psychological pain or they may opt to engage in E-Coaching when they want to work collaboratively with a Coach to help them reach personal goals not related to emotional issues (e.g. to learn stress management skills, make a career change or learn teenage parenting skills). Life Management Systems also provides other services that include training, management consultation, substance abuse evaluations, and policy review.

HealthSpan E-Coach

Offered in partnership with Hummingbird Coaching Services, HealthSpan delivers a unique personalized coaching tool to assist members in achieving their specific health and lifestyle goals. Unlike counseling, coaches form collaborative on-line relationships with members by assisting them with effective goal setting, presenting options to reaching success, and on-going support and motivation through confidential communication. Members benefit from the advantages of e-mail coaching through the ability to communicate and access resources at any time according to the member’s schedule. All coaches undergo an intensive training program including timely quality reviews. The HealthSpan E-Coach web site also provides a variety of tools including tracking mechanisms, a resource library, daily messages and much more.
